Consultation

Service Description

In the initial consultation I will be reviewing all of your completed paperwork including: medical history, health goals, concerns, symptoms, lengthy questionaires, any previous lab work and a 3 day food log. This appointment is mainly for intake so I can learn more about you, the state of your health and how I can best help you. After we have completed your initial visit we will schedule you for an included follow up visit for your report of findings. Report of Findings Follow up Visit - In this visit we will go over what I found, how I can help you and what your personalized program entails.
